Instrumental perforation of the oesophagus into the pericardial cavity.

چکیده
A 66 year old man with no previous oesophageal symptoms developed acute dysphagia and vomiting shortly after a large meal. About 16 hours later a large piece of meat was removed from the oesophagus at the 30 cm level under general anaesthesia through a rigid oesophagoscope. The surgeon who performed the endoscopy explored the distal oesophagus for strictures and noticed that the oesophagoscope would not pass beyond 40cm. A few hours later the patient developed moderately intense chest pain and surgical emphysema of the lower neck. He was given penicillin and gentamicin intravenously and transferred to the Regional Cardiothoracic Centre for further management. The patient had a heart rate of 105 beats/min and a

Instrumental perforation of the oesophagus and perforated duodenal ulcer.
Instrumental perforation of the oesophagus is a serious and, unfortunately, not a very rare accident. The occurrence in addition of perforation of a duodenal ulcer constitutes a situation which is rare, difficult to diagnose, and fraught with great danger to the patient. Simultaneous perforation of multiple peptic ulcers has been described in several reports since that of Finney (1900). As pept...
متن کاملMorbidity and mortality of oesophageal perforation.
Thirty-three patients with oesophageal perforation seen within a 20-year period are reviewed. Endoscopic perforation in patients with neoplastic disease of the oesophagus carried a 100% mortality despite early diagnosis and immediate resection of the growth. Only two deaths occurred after instrumental perforation of the thoracic oesophagus for benign pathology in patients who were in the extrem...
متن کاملAccidental left ventricular perforation during coronary angiogram.
Myocardial perforation (MP) is an uncommon entity which occurs most commonly in the setting of acute myocardial infarction. We report a case of accidental left ventricular perforation during a routine coronary angiogram. A 55-year-old male, known diabetic with moderate renal impairment, non-hypertensive, suffered an acute inferior wall myocardial infarction for which he was thrombolyzed with st...
متن کاملAn Unusual Cause of Cardiac Tamponade during Cardiac Catheterization Study
Introduction. Catheter-based diagnostic and therapeutic procedures are rapidly advancing. However, catheter related complications sometimes become life threatening. Cardiac tamponade is a rare but serious complication of this procedure. We have summarized one cardiac tamponade pejoration due to secondary coronary vessels laceration by the implanted pericardial drainage. Case report. A 4-year-ol...
متن کاملPyopneumopericardium as a complication of oesophagogastrostomy.
Pyopneumopericardium is a rare entity and is even more infrequent as a complication of benign gastric ulceration following oesophagogastrostomy. The usual course of events is the perforation of a gastric ulcer with subphrenic abscess formation and dissection through the diaphragm into the pericardial sac. The cases reported in the literature are associated with extension of carcinoma of the lun...
